Grounded in Science, Built for Flight: What Dry Needling Does for Your Trigger Points


Whether you're holding a crosswind approach at 200 feet or asking a 1,200-pound animal to collect and bend through a lead change, your nervous system and your muscles are doing the same essential job: stabilizing a moving system under load. Over time, that job creates trigger points — and trigger points are quietly expensive.



In my practice, I see two populations that look completely different on the surface — aviation professionals and equestrian athletes — and yet arrive with almost identical complaints: a deep ache along one side of the low back, a hip that never quite unwinds, a knot between the shoulder blades that shows up before every long flight or every derby. The common thread is asymmetric, sustained loading. The fix, more often than not, starts with dry needling.



What a Trigger Point Actually Is

Physical therapists Janet Travell and David Simons defined a myofascial trigger point as a hyperirritable spot inside a taut band of skeletal muscle — a small area that stays contracted long after the task that created it is over. [1] That taut band restricts blood flow, irritates local nerve endings, and refers pain to places that can feel far removed from the actual problem: a psoas trigger point that mimics a groin strain, or an upper trapezius knot that presents as a headache behind the eye.


For a rider, this often shows up as a hip or gluteal trigger point that quietly throws off symmetry in the saddle. For a pilot or flight attendant, it's frequently the cervical and upper thoracic muscles, loaded for hours in a fixed, forward-leaning posture with almost no variability in position.

Why Dry Needling — And Not Just Stretching

Dry needling uses a thin, solid filiform needle inserted directly into the trigger point, which is mechanically different from foam rolling or static stretching because it reaches deep into the muscle belly rather than just the surface fascia. When the needle contacts an active trigger point, it typically produces a local twitch response — an involuntary muscle contraction that appears to interrupt the dysfunctional feedback loop keeping the band contracted, while also triggering a local release of anti-inflammatory substances and endogenous opioids that help reset resting muscle tone. [2]



Why this matters in the cockpit and in the irons: both populations rely on fine motor control and proprioception — a pilot scanning instruments while making small, continuous control inputs, a rider making half-halts through the seat and leg. A muscle stuck in a contracted, guarded state degrades that precision long before it becomes painful enough to notice consciously.


In the Treatment Room

Here's what dry needling actually looks like in session — sometimes on its own, sometimes paired with light electrical stimulation to enhance the local twitch response and keep the muscle engaged throughout the treatment. Sessions typically target the upper back and shoulders in pilots and flight attendants working long duty days in a fixed seated posture, the forearm paired with e-stim to keep the muscle actively responding through treatment, and the hip and gluteal region in riders working through asymmetry in the saddle.




What the Research Actually Shows

The evidence base has grown quickly, and it's worth being precise about what it does and doesn't say. A 2023 systematic review and meta-analysis of dry needling for neck pain from myofascial trigger points found meaningful short-term reductions in pain compared with sham treatment. [3] Earlier work by Navarro-Santana and colleagues, summarized in a 2024 payer policy review, similarly found dry needling reduced neck pain immediately after treatment and at one-month follow-up when compared with sham or waiting-list controls. [4]


For low back pain specifically — relevant to nearly every rider and every pilot who's spent a career in a seat — a 2023 systematic review concluded dry needling is an effective adjunct for both acute and chronic myofascial low back pain, while noting that long-term outcome data is still limited. [5] A separate randomized, single-blinded trial reported moderate evidence for short-term pain relief with both static and dynamic needling techniques. [6]


The sports-specific data is directly relevant to both of my niches. A 2024 systematic review of dry needling in overhead athletes with myofascial shoulder pain found consistent short-term pain reduction, though it flagged a need for more rigorous trial design. [7] A broader 2025 evidence-gap review across 24 studies and 580 athletes found dry needling generally more effective for reducing pain and muscle stiffness than for directly boosting athletic performance metrics [8] — an important, honest distinction: needling clears the interference so training and rehab can do their job, rather than acting as a shortcut around conditioning.


One controlled study in elite youth soccer players is particularly relevant to riders: four weekly sessions of dry needling produced measurable improvements in hip flexion range of motion and hamstring/quadriceps muscular endurance that persisted at four-week follow-up. [9] Hip flexor tightness has also been identified as one of the few well-supported risk factors for low back pain in athletes generally [10] — which tracks with what I see constantly in riders sitting asymmetrically for hours a week and in pilots folded into a seat for entire duty days.



For Aviation Professionals

  • Targets cervical and upper thoracic trigger points from prolonged, fixed seated posture

  • Addresses hip flexor tightness from hours in a rudder-pedal or jump-seat position

  • Helps reset guarded shoulder and scapular muscles from repetitive reach-and-scan patterns

  • Fits irregular schedules — a fast, targeted session between rotations



For Equestrian Athletes

  • Addresses gluteal and hip asymmetry that limits an even seat

  • Releases lumbar paraspinal trigger points from repeated flexion-extension in the saddle

  • Targets adductor and psoas tightness that restricts leg aids and following the horse's motion

  • Supports the core stability needed for effective half-halts and collection



What a Session Looks Like at Skyline

Every dry needling session is built on a full movement assessment first — I want to know why a trigger point developed, not just where it lives. From there, treatment fits into the same three-phase framework I use across all of Skyline's programming:

Dry needling isn't magic, and the honest research reflects that: most of the strongest evidence is for short-term pain and stiffness reduction rather than a permanent, one-time fix. [7][8] Used consistently as part of a broader plan, though, it's one of the most direct tools available for interrupting a trigger point cycle that stretching and rest alone haven't resolved.
